Should I be accepted by Electric Mobility, any misrepresentation or false statements contained herein may be considered cause for possible contract termination. Electric Mobility has my permission to obtain all necessary information from the references I have listed, or any other sources concerning my prior employment, personal history or credit standing and I release all parties from any possible damages resulting from disclosing such information with or without prior written notice to or from me. I reserve the right to know the names and addresses of any investigative agencies used in order that I may learn the information contained in any reports furnished to Electric Mobility.

I understand that the issuing of any contract with Electric Mobility shall be contingent upon the successful completion of a background investigation. My agreement authorizes Electric Mobility to conduct such investigation including, but not limited to, criminal records and consulting the National Practitioner Data Bank, Cumulative Sanction Report and the General Services Administration's List of Parties Excluded from Federal Procurement and Nonprocurement Programs.

I understand this application does not constitute acceptance of any kind. If accepted my contract may be terminated with or without cause and with or without notice at any time by myself or the Company within the limits of the Independent Contractor's Agreement which will be made available to me upon mutual agreement.
